Description: I need a simple mobile app built, which works on a 7" android tab and standard mobile phones

The app is used to collect text data via text fields, radio buttons and image fields. The image field has access to camera / file system saved image. The app user add these data and saves the data.

There should be a backend where the entire data could be exported to an excel file
