Tool will be used for
-client referrals
-case note records
-interventions
-any statutory actions taken under the Public Health Act 2016.
Existing tool is written in the 90's on MSAccess, but the three users are very likely to want a similar layout - so not default Bootstrap/Material design, but a much tighter layout
-capture demographic and statistical information
-search any parameters or key words in the database/case notes
-extract data
-formulate reports
-formulate and measure KPIs
-upload documents (e.g. Briefing Notes, letters, referrals, emails, reports, Public Health Orders)
-set reminders and tasks for individual patients
Since this is a medical application carrying patient data, some general requirements also exist
* Data must be encrypted by the client, using a shared key, that can be replaced (or kept for historical reasons)
* Data needs to be encrypted in transit as well as rest on the server
* Authorization needs to be robust (server side)
* A robust audit trail needs to be kept, with the preference of no ability to delete data.
In addition, the users wish to be able to display Alerts - these alerts derive from HL7 messages within the organisation.
There are some conflicting requirements here:-
1. Client-side encryption normally means no ability to do full-text searches
2. Encryption of PII data means the server is unable to identify a user to receive HL7 messages
Fulltext search on client side can be resolved to a degree via https://github.com/lucaong/minisearch with a server side process to temporarily store attachments for text extraction (receive attachment, identify type, trigger appropriate tool to extract text, load text into couchdb Doc, delete from disk - encrypt at rest can be done at the server level to avoid breaches for this even though it does expose the data to admins)
This approach may need to occur for HL7 interfacing as well - ie. list of medical record numbers is stored in a table, with this data being collected and made available in clear-text for client to receive and then delete from server
It is expected that the solution would utilise react on the client side with pouchdb/couchdb on for case storage. Reportable fields should be able to be stored in couchdb un-encrypted as they would not be identifying data. On the server side laravel can manage interactions (and preferrably provide the couchdb interface as well so that it is not exposed outside of an authenticated session). A backend ETL process can handle migration of data from couchdb to mysql, another process can handle text extraction of couchdb attachments
Reporting can be handled server-side using the easyquery tool within laravel environment - ie a separate module, but GUI layout needs to remain the same.
Server side also needs to be able to run as at least an HA pair, or a cluster.
