- Description
- We have build a custom application what is built using PHP (laravel) & MySQL. We currently have a registration form what we collect information manually with a pen and paper. We need this form creating within the application. (this document is attached)
The application has multiple users (I can define the roles and capabilities later on if required)

We currently have built a form already what collects some of the information. This is like a pre-registration form what collects name, phone number, email address of the candidate before they login to the system.
The form what needs building is additional information what the candidate can fill in. You will see in the form attached some of the fields already duplicated within the application as mentioned above, like name, email address, phone number etc. We do not need to duplicate this data input. The candidate can be assigned a mentor within the system currently and the mentor will need the ability to add and change the form on behalf of the candidate if they need to.
Super admin
Within the super admin area (what controls and oversees everything) they need the ability to assign a cost to each question what the candidate will fill in.
For example
Question A = $10
Question B =$50
Question C = $24
It is absolutely critical that you use normalization within the database. This is a process of structuring a relational database in accordance datasets. This is how the database is currently structured. We have done this to reduce data redundancy and improve data integrity.
Once this project has been completed with we have more on going development what will need completing. We want to build a long lasting relationship with the development team we choose as this is a growing application and will require more development in the future.
We can give you access to the server. We currently use GitHub for all our version control.
Everything will become alot more clear once you have access to the server. It also may be eaiser explaining this over Skpye? as it can be difficult to explain in a written format.

MORE WORK ONCE COMPLETED
This data at a later date will be pulled into a dashboard and they will be able to see a snapshot of all costing activity from the candidates. We already have a dashboard currently what gives insight into areas of the system. This will become clear once you see the application.
We would like to build out more graphs and charts on this data what the candidates have inputted.
